Povidone iodine is one of the commonly used agents in the perioperative period to prevent infection and is considered to be safe. We report a life threatening cardiac event following the intraosseous use of povidone iodine. Intraoperatively, povidone iodine was used repeatedly and forcefully intraosseously. This probably led to systemic absorption owing to increased vascularity at the irrigation site due to infection. Owing to serious consequences of absorption of large amounts of povidone iodine into blood circulation, care should be taken while irrigating raw areas during surgery. Povidone iodine should be used in a concentration of 1: 10 as higher concentration leads to toxicity while higher dilution may decrease its antibacterial action. The duration of flushing or irrigation should be limited and should not be forceful.
